I have lived in various cities and towns around NY (outside of the metro area). I just had a friend that was transported 6 miles to a hospital and the bill was a $1,000 - and that was in the fire department ambulence. We are not in a large metro area, but is a census MSA area, with two large medical centers.

The closest I have ever heard of an ambulence being “free” even when its a tax supported service is if you are on Medicaid/Care. Everyone else pays, and it may or may not be insurance covered.
