It is avoidable if you don’t do your beta testing with your customer base in uncontrolled conditions. Ultimately it’ll take a hell of a lot longer than Elon is willing to wait to work out the kinks. But it is definitely avoidable.

Good point, but if autonomous cars are going to improve on human drivers, then they probably need to avoid human-type errors. 

Which is why most (all?) the competitors are not marketing their LiDAR systems as fully autonomous systems. But camera-based tech is no closer to achieving full autonomy. “Tesla isn’t there yet” is a yuuuuuge understatement.
