The fact that Germain says in the review that there’s never any explanation given for the alternate universe would suggest, if the character is supposed to be in a coma, that’s never revealed in the movie.

I hate to break it to you, but, as much as I loved Agent Carter and would have liked to have seen more, it was cancelled because its ratings weren’t that great and it was more expensive than usual to produce due to the period setting, not because of some misogynistic conspiracy.

Strictly speaking practicing medicine without a license is a state crime, but depending on what all you do while practicing medicine without a license you can easily commit a bunch of federal crimes too (most immediately in the realms of wire fraud, mail fraud, controlled substances, etc.).
