I think the plastic top section presents the fatal flaw in this particular design. I think most of them use metal for both of the pressure containing pieces, which should be less prone to this failure mode.

I found it odd that the IG post pointed out that the cartridge was sealed. The vessel was obviously pressurized, so clearly another cartridge’s contents had already been expelled into it.
