I think you nailed it. I remember him saying he couldn't make Serenity "matter" without killing off a character, and I feel like he puts that mindset in a lot of his writing. Except like you said his deaths are basically character hostages.

It might be that he jokes without changing his tone, making it hard for people to read if he's kidding or not.

The process of planning and writing for Martin is incredibly interesting to me, and I think you're spot on here. I don't think he's figured out a way to make the middle act work after scrapping the five year plan.
