I have PCOS as well, and I do have effects due to the increase in androgens. I can grow a beard if I want to, to say the least. Oddly enough, I still have a regular menstrual cycle (It leveled out as I grew older). So, affects are definitely dependent upon the person.

Considering that PCOS, a hormone imbalance where androgens are raised over the norm, affects 1 in 10, the IAAF is going to have to do a lot of testing if they want to play this game.
