A worse outcome than one positive outcome doesn’t mean negative. That you don’t get that fact doesn’t change it. If someone offers you $20 or a root beer float, the root beer float isn’t magically a negative just because the $20 is something you prefer.

The choice is A) change nothing or B) get a healthcare procedure free of charge (and also get a reduced sentence). Neither of those is anything worse than a neutral. The fact is, this simply offers them an additional option that didn’t exist before, and nothing is done to them for not choosing to take it. They get to continue on exactly as before and exactly as if the option never existed.
