That’s actually on purpose. When Hayden Fry took over, Iowa had been a losing program for a long time. He decided to have the unis updated to look more like Chuck Knoll’s Steelers, given their success at the time.

His official cause of death was brain penetration by shrapnel, but he did suffer a basilar skull fracture just like Ratzenberger and Dale Sr., so he may have died even if the shrapnel didn't hit him. HANS and a canopy might have allowed him to survive.
