There isn't one. BMI is not a diagnostic tool. No doctor only looks at BMI and bases all patient care around that. People love to point to outliers like body builders and skinny fat people because it makes them feel better.
I don't think it's fair to blame obese Americans, because clearly the deck is stacked against them. But I do think that we should acknowledge their agency in their own situation. And I think we should acknowledge that there are non-food cultural factors that contribute to the problem.
