They had a scene yonks ago with Romana picking her appearance. It was basically a retcon to explain why she looked exactly like another character, because they'd hired the same actress to do both roles.

I find it more curious that given all of humanity and human history, the Doctor always turns up white.

I actually agree! Camera work can make or break a show. Top Gear is sublime because the camera work is so damn good; the Dresden Files was nauseating because the camera work was so bad.
