Everything you said is very true. Albeit, I haven't seen many of the less commercially approved porn you're talking about. I have seen a lot of tears, choking, piercing, complete degradation, ect that were in commercial shoots. Things that I found extremely disgusting personally, but I believe was truly consensual.

I think 99% of porn that's easily accessible is regulated on sets with recognizable porn actors, including BDSM/Rough/humiliation porn.
