The reason "I could care less" really means "I couldn't care less" are because it was used sarcastically for long enough and in a pervasive-enough way that it is now an accepted meaning. If I say "I could care less", I don't have to say it

Actually "I could care less" is an example of an English phrase that means the same thing as its antonym(ic phrase, in this case).
The current meaning of "I could care less" stemmed from sarcastic use, but is now perfectly interchangeable with "I couldn't care less".
