That’s really just a brand difference.  Both SNES and Switch (and probably other Nintendo controllers) use the XABY layout.  All Xbox controllers use the YBAX layout.

Sadly true. In most of the cases the cause is assumed based on various bits of data (previous medical issues, family history, circumstances of their passing), but to confidently determine the cause would be either impossible or too difficult/costly to be worthwhile.
