Well, I think Ramsay wanted Sansa to think that. But it could easily be a larger network of people he wasn't aware of…with Brienne indirectly being an extension of Sansa's protectors.

On the other hand though, how would we expect Ramsay to treat Sansa? Of course, it's going to be terribly. I suppose they could've done it off screen. But it's unrealistic to think he wouldn't have been abusive toward her considering how he treats everyone else.
