My wife works at a cable channel and it's a mess. Sometimes the shows add the bars, sometimes it is done at the network, and the bars are never the same size or color. When it finally gets to broadcast, it may look fine on cable, but not on Dish or DirecTV. One engineer made them refeed a show because it had no bars.…

In most cases like these it usually turns out that the officer was not properly trained for the situation and had to use their own judgement. Cops, like doctors, lawyers, programmers, and bloggers can sometimes use bad judgement. It's just that most other people don't have tasers and handcuffs.
