Yeah, hiatial hernias happen most commonly in the elderly and in obese people. And to my knowledge aren’t typically known to cause weight loss. Sounds more like anorexia nervosa.

Am a 36 year old man with a mild hiatal hernia. Sometimes it hurts really damn bad after a large meal. If it were severe, I can see it causing someone to choose not to eat rather than feel that pain, but you are correct that he doesn’t have the look of most hiatal hernia folks.

I have a hiatal hernia and am little younger. I’m normal weight/thin-ish and I drink less coffee to manage my symptoms. Sometimes I throw up if I’ve had too much tomato sauce and/or whiskey. It’s an acid reflux thing.
