I only have the vanilla version but was able to kill all but one idol with the wedges in the level prior to beating the other boss which you should totally go and do in any case because it's an amazing fight. One of my favourites in the game.
You're probably right with it being an anglicised version of her name. I'm not sure how my name would translate with the "th" at the end and gave up learning Japanese pretty early on so I never found out. I'd be interested to know if anyone speaks it.
Her name confuses me as written as there is no "l" consonant sound in Japanese. It's only used in foreign loan words and they just use the "r" sound instead so I guess it's a stage name. "Ree la" is my guess.

That happens in the book (90% sure it was the same breed of dog) which was published in 1975 so predates UK involvement in Afghanistan, unless there's another war there I'm missing.
