Sounds to me like they are maybe thinking of it as not an extra $10 4G fee, but more of an "hey its the iPhone and we all know you want one, so we are going to add $10" fee....

Yeah, that is kind of odd....for other phones like my EVO that $10 is supposed to be an add on for the 4G connection.

Being someone that does not travel very much at all internationally, that sounds like a lot of hassle. Sounds like the easiest, simplest solution is to stop in a store of whatever country your visiting and buy a throwaway prepaid phone.

I have not heard that myself, being a gold subscriber, definitely have not heard from xbox they were raising the annual amount. Only change I know of is the only one they have ever done in the history of the service, a year or 2 back when it went from $50 to $60.
