I know more than one person who claims a religious exemption for their child who are atheists or have no real defined religion. They took that exemption because I believe they just had to write a letter to the state rather than the medical one which requires a reason from a doctor.

They and other countries don't pay it because they view it as a local tax from which they are exempt not because they think it's fun to not pay it. The US is not the only country that feels that way.
Foreign diplomats do that in DC as well. I watch cars with diplomat plate violate traffic laws all the time. Parking tickets are not on par with lying on a visa and paying someone well under minimum wage.
