Depends on the rate of change. If the climb rate was high (as you’d expect in a jet) and the cabin altitude rose too quickly I can see how the rapid change would be painful. 

I fly a King Air so I’m familiar with the process. It’s the rapid rate of change that causes problems, so if the cabin wasn’t pressurizing it wouldn’t be a big issue as they probably held at a lower altitude to sort things out. But if they climbed quickly with the cabin altitude still high I can see now how that would

It probably took that DC3 a lot longer to get those altitudes than a jet airliner, giving your ears that much more time to equalize.

The lack of pressurization means the pressure *inside* the ear is much higher than outside. The pressure imbalance is going the other way. 

But what about the differential between the cabin and behind the eardrum? As elevation goes up, if cabin doesn’t pressurize as airplane rises, cabin pressure goes down relative to earlier in the flight and ear pressure stays the same.
