The Co-60 source for teletherapy is a metallic cylinder containing Co-60, the Co-60 itself being a metal ingot. No inhalation, no dust, etc. They would have to be in proximity to the thing. Also, the thing is very hot (thermally, due to decay) making it almost impossible to hold and probably making a radiation-idiot…

Up close, still a risk. Employed in a dirty bomb and dispersed over a wide area, though, much weaker than a "fresh" Co-60 source. Still…
