Like others have said, you don't have to use a shield at all. I used to be exclusively sword/shield, but lately I've been leaving the shield slung away in favor of a spell in my off-hand. There are plenty of weapon and/or spell combinations that you can arrange however you want.

To specify which hand to equip something in, use the triggers (or mouse buttons on PC, I think) instead of A (or Enter). The only restriction on which hand you can use something in is with shields, which must go on the left hand.
