I had one company years ago with a very crappy plan, which had sky-high premiums on top of only paying 50% after a certain amount. It’s logistics - all of the extra baggage and more passengers to process. It’s a lot. They also have to verify you have a valid passport/visa too because they will get fined if they let you on the plane.
