Its actually probably familiarity with the flag that caused this goof. If they weren't familiar they would have almost certainly just looked up the PR flag. But because they were familiar they just chose the flag from memory, albeit incorrectly.

From my experience in game development, this was likely done by an outsource team. These kind of assets tend to get outsourced. Sure a dev finishing the room might have noticed it but having been in QA for many many years, I likely wouldn’t have noticed :(
