Yeah, that’s a highly specialised field that people would enter because they want to perform that type of surgery. Maybe it allows other staff members to opt out, but they’d still have taken a job in that department.

There is a way to muddle along a sort of middle ground. Firstly, if you’re performing gender reassignment surgery, you’ve entered a highly specialised field presumably because you want to do just that.
