GM 400CID Gen 1 small block running through true dual exhaust with some sort of glass packs. It's your basic redneck special exhaust (true redneck style would exit right after the glass pack)
I would put line wrenches on the list. Nobody ever has them and using regular wrenches can easily strip brake hardware.

I would imagine the turnoff also allow for much better torque. Combine that with 6 and 7 speed transmissions an you have a small displacement with enough torque for tall gearing.
My 2006 Scion xA 5-speed pulls in a regular 33MPG and I don’t baby it. I can get 35-37 from time to time if I try hard.
I seem to remember the biggest issue was improper torque on the head bolts leading to head gasket failure.

I believe Astros are even tighter. At some point, GM stuffed a 454 into the G-Van body. Not sure if they redesigned the doghouse to fit it.
Not really. The motor sits far enough back that the headers are about floor level (wait that makes it a Front model engine like an old school RX-7)
