That’s not how tax brackets work. Only the money in the 28% bracket is taxed at 28%. Everything else being equal, your neighbor and you pay the same in taxes on the first 125k.

Bell was drafted two years after that CBA was negotiated, so he didn’t really approve it. At no point has he been able to set a price for his services.

There’s nothing in the CBA that says the player has to sign the franchise tender though. They are absolutely free to hold out. Bell is operating entirely in the bounds of the CBA as well.
