Many commands, especially for things like takeoff clearance, are supposed to be repeated back to the controller. But at the rate they’re pushing planes, and given the technical limtis of analog VHF radio, those readbacks can often get lost. Dangerous assumptions were likely made here.

This is a pretty neat idea, but I am also of the mind if your kids can’t behave DON’T BRING THEM.  If that mean’s you can’t go out for a nice dinner, to bad.  And honestly that applies to bringing kids anywhere.

The irony is that a guy that just lost a defamation lawsuit, and got slapped with another one for telling lies, is simultaneously suing another company for defamation for telling the truth.
