It's still "breast cancer" elsewhere in the body if it _began in the breast and subsequently spread elsewhere_. By removing the breasts, people at high risk reduce the amount of breast tissue which can become cancerous, and if there's no primary tumor, there's no metastatic breast cancer.

I enjoyed Dhalgren a lot; or I did the last time I re-read it, 15 years ago or so. Dune, as well. Foundation and 1984, well... I read them, but didn't love them. And Cryptonomicon still has a bookmark about a quarter of the way in; it's in my active to-be-read pile. Pretty sure I haven't attempted the others yet.
