Well, some cheaters are sociopaths. Some are narcissists. Most are neither, they have no mental illness to justify their behaviour, usually just an emotional rationale - disappointment, frustration, opportunity, failure of judgment.
That is how it plays out, generally. Even the people who know them probably don’t know, or don’t want to know. They could even be mutually abusive, it isn’t that uncommon, but scuttlebutt is no more certain than anything else.
