I think that would be akin to being a woman, for example, with very masculine facial features who could “pass” for male.

They’re assuming you’re a woman, and that if bathroom attendance was based on biological sex then you could end up with a trans man (i.e. born biologically female and transitioned to male) in the bathroom with you. Which, depending on the extent of the transition, could appear to be a biological man in there.
