It is entirely possible that she was ignorant of the outcome of sex but it takes a powerful amount of denial to deny that *something* noteworthy is going on. For better or for worse.

I found out I was pregnant at 32 weeks so I don't think it's inconceivable that she not know but she had to have known she was having sex in some capacity. I'd wager a good deal of her not knowing was denial. Denial is a powerful motivator to ignore obvious facts. I may be speaking from experience :)
