It's not clear in the books either. I'm going to go ahead and assume that the show consulted with GRRM and is the authority on this, and that it is spread person-to-person when infected skin directly touches uninfected skin.

I miss the Frey pie so much because it's one of my favorite things in the series, though it's not my main complaint because I kind of get why they might not want to include it.
