Even with E. coli O157:H7, you still have some people with relatively mild presentations and asymptomatic people. I really doubt this is E. coli O157:H7, given the numbers and spread, though. It makes more sense if it’s a variant that can cause very severe illness in a minority of cases.

Most people who get e-coli simply stay home with the “stomach flu” for a few days. Others go to the doctor, but the doctor doesn’t report it. It’s a very small fraction where it’s actually reported AND linked to an outbreak.
