I don’t understand why people always assume that reduced drug prices in the US won’t result in increased prices elsewhere. None of this happens in a vacuum, and it’s not our job to subsidize drug costs for the rest of the developed world*.

There’s always the public option route.  Offer a public option and just make steadily harder to compete with.  Let private insurance companies broker it and manage it in exchange for being regulated.
