Not all of Scotland is a graveyard. The encounter the trophy is for happens after you discover Avery’s grave, solve the puzzle, then come back out into the graveyard. If you can make it through that encounter without being noticed or killing anyone, the trophy unlocks. It’s definitely a challenge, but doable.
