I don’t think it’s fair to categorize this as not being concerned with story. Early game were emergent, for one. Two, we didn’t use half the stuff in the DMG and the author agreed we shouldn’t. It always was a toolkit. Over time, that notion has been lost. That said, I never used the disease table.
