Actually, changing shoes will help with a lot of her owie parts! When you're pregnant, you're center of gravity is totally thrown off. So, back pain, headaches, hell maybe even stomach pain because she will be working so hard and using her ab muscles differently to keep from falling over.
For the first year in forever we are actually getting money back. After 10+ years of freelancing it was a shock to the system to get a refund rather than having to pay out the nose since last year was my first full non-working year. Self employment taxes BLOW!

I notice that several the cities in the Top 10 are "destinations" where a significant proportion of the jobs are likely to be in hospitality, tourist/service-oriented businesses — Tampa, Miami, Vegas, Sarasota. Not sure what that means exactly but I have a feeling that has a lot to do with the lower wage gaps.
