@endrew: They did plenty of real-world testing. Unfortunately, due to their policy of secrecy on unreleased products, the testing was done using a case to disguise it as a 3GS.

The reception problem is not a design flaw. It is definitely a manufacturing error, but not one that affects every phone. I've tried to replicate it on several friends' phones and have been completely unable to do so.
