My personal preference is to have my DNS provider do the work of maintaining a site blacklist so that I don't have to manage the hosts file manually. That in tandem with a good Ad blocker and a few manual hosts entries works for me.
Yeah, that advice is intended for home users (and only if they see a major slowdown). It's not going to be appropriate if you're part of a domain, like at the office, but then again I wouldn't expect you to be maintaining a huge hosts file inside a corporate domain. On some Windows platforms (XP/Win2k) it is alleged that a large hosts file can cause problems with the DNS Client service, so the recommendation is usually to re-configure that service so as not to run.
Some people get mildly upset over cosmetic (UI) changes, but they get positively rabid when basic function that they've used forever stops working completely and is barely functioning 6 months later.
