Teams do practice kickouts, and have a designated package like this where they’ll have the kicker and punter back to receive a kick and boot it out again, but the kickout going straight back to the opposing kicker was just a freak accident.

That’s an *offensive* try safety, which has only happened twice. A *defensive* try safety, which would end with a player on the kicking team being tackled in the end zone at the other end of the field, and result in a single point for the defending team, has never happened.
