It might be safe (ish) but I wouldn’t call it legitimate. That setup is not to code and would 100% fail inspection. But I’ve seen worse.

yes, i have used as many tandem circuit breakers as the box is rated for. The house is also wired with common neutrals so there is even less flexibility to reorganize the box since both hot leads of the 12/4 wire need to be on different legs of the service.
