I would guess it's because women in colder climates double up on their footwear. Wearing warm snowboots into the office and then changing into a kicky little pair of pumps is pretty much par for the course between November and April where I live (Western NY state).
The line is whether or not they could manage to commit suicide without an assist, I think. Laws allowing medically assisted deaths are really meant to protect the "assistants", given that if it works out as planned the "victim" is dead and beyond legal repercussion.
