Always look at the bill. There could be an assortment of errors, including getting the bill for the table of six next to you that had appetizers, a few bottles of wine, and dessert.

So now when I pay cash I make sure some of it is sticking out of the little folder thingy so they know that I paid.

I once left cash on the table (in the folder thingy) at a pretty nice steakhouse in Biloxi, MS and the waitress came tearing out of the restaurant to accuse me of skipping out on the check. It was weird and embarrassing even though I did nothing wrong.
