I think it’s great that you go to such lengths to assuage people’s fears - but here’s my take on it.

I want to clarify that the reason an online questionnaire is not the same as an in-person visit with the doctor is because people will accidentally leave amazing amounts of information out of a form that a little prodding and asking of the right questions.
