I'd imagine it landed and that's where the broken hand came from. It's actually pretty easy to break a hand punching someone hard in the head. That's part of the reason boxers and MMA fighters wear gloves. It's more for the hands than the head as you can easily break a wrist or hand hitting the skull hard and off.
