We had milk in bags at my university coop in the US. But yeah, for individual containers, I've mostly had it in Peru (not just cow milk, but soy milk as well).

A frozen pizza costs 70 cents (but I usually add some more stuff, not including nutmeg), while I can get a fresh (crappy) restaurant pizza for €1.80, so the economics don't really make sense. But I still have frozen pizzas fairly often.
