The difference between the fellow's withdrawals and your scenarios is the fact that his withdrawals left an electronic trail. Someone diving into a pallet of money or stealing cookies from an unattended jar are less likely to get caught, than a guy taking money using his name.
If you went to a regular ATM, there would be a limit to how much you can withdraw. I assume the fact that he was using the card at casinos is how he got so much. Of course, he probably could have bought gold, diamonds or something tangible, but I don't see how you could pull that much money out of regular old machines.
