Both. I've lived and frequented comic shops in places with populations of 40k and 600K, and between. There are societal/cultural factors of course (for instance the Comic book store with sports memorabilia and collectables, were much better than stores that just sold comics... also, the comic store near a college

Not all, but many.
I think the problem isn't even as much the misogyny as just the fact that many comic book employees are dicks. I'm not sure why this seems to the be the only retail places where customer service is frowned upon.

It's wasn't a couple of stumbles....every single thing Claremont wrote post 1992 was absolute garbage. He kept getting X-men work based on his resume' and it was always horrible.
