Women come in all shapes in sizes. Don't criticize heavy people (or even mention a national problem with obesity), lest you be accused of "fat-shaming," but feel free to criticize thin or skinny women, especially if they make money off of their appearance. I'm pretty sure the frequent beatings, general lack of any food, and drug resistant strains of TB and HIV are a bigger problem. Russian prisons have their own strains of those diseases, only found in the prison systems, and they're some of the most quickly progressing, deadly forms of the disease.

See, that's a normal reaction. I don't like the shirt, I'm not buying a shirt. To be offended by something as silly as this t-shirt is frankly doing a disservice to all the actual offensive things in the world.
